Introduction

The Ministry will be conducting a survey annually. Thank you for taking part in this survey.

Given that some Safe Areas have been established as preventative measures, some of the questions in this form may not be relevant for your Safe Area. If this is the case for you, please write N/A in the free text area or leave comments as needed.
