Public consultation for the review of the End of Life Choice Act (2019)

This consultation asks some general questions related to the End of Life Choice Act. A number of the questions reference clauses in the Act. If you would like to review these clauses they can be found on the NZ Government Legislation website.

You do not need to answer every question. Questions that are compulsory have the words ‘required’ in the text.

We recognise assisted dying is an issue on which philosophical, moral, religious, ethical and clinical views are divided. While we understand there can be strong views on issues related to this, we will not accept submissions that include abusive or inappropriate content. 

About you 

We ask for your name at the beginning of this survey. The names of all submitters will be included as an appendix to the final summary report, which will be provided to the Minister of Health. Your submission will not be published alongside your name as part of the summary report.    
 
Any material directly referenced in the summary report from individuals will be anonymised, though the names of organisations may be referenced.  

Your full submission will be subject to requests made under the Official Information Act (even if it hasn’t been published). If you want your personal details removed from your submission, you can let us know at Question 5 below.
