Stocktake of activities progressing elements of the New Zealand Framework for Dementia Care - GPs and Practice Nurses

Overview

This stocktake responds to the need for New Zealand to better understand service delivery for Dementia – in particular strengths, opportunities, gaps and barriers.

The survey is being targeted to nine different groups across our district so that we can get a comprehensive picture of activities underway. You are receiving the survey targeted to GPs and Practice Nurses.

For ease, the questions have been grouped around the New Zealand Framework for Dementia Care (the Framework) as this provides DHBs and health and social sector agencies with a guide for developing their dementia care pathways (see below diagram).

It is over five years since the Framework was released and it is timely to look at how work is progressing.

DHBs are encouraging open and honest replies so that we are able to highlight the gaps, as well as identify models that are working.

The information collected through the stocktake will be used by the Ministry of Health to help inform advice to the Associate Minister of Health (responsible for aged care). It should also enable DHBs and the regions to identify future priorities and to share information, learnings and best practice.
